So, we've already talked that there are a number of different models of defibs on the market. So we have actually changed defib now, we are going to use a completely different one and you will see that the talking, the words, the sounds and everything else is exactly the same because it's the European guidelines. So the first thing we're going to do is turn the unit on, listen to what it says.

Training scenario. One, adult patient.

We'll then... We don't have to wait for it to talk, but we can.

Call for medical assistance.

We pull the pads out. Soon as the pads are out.

Remove clothing from the patient's chest to expose bare skin.

We expose the chest which we have already done. We take the two pads and you will see that on the pads...

Pull green pad to remove pads.

It shows you where to place each pad. We take the first one off...

Peel pads from plastic.

And we place it on the top right pectoral muscle. We take the second pad off.

Apply pads to the patient's bare chest. As shown in the picture.

And we put it on the left side of the chest. Like so.

Press pads firmly to the patient's bare skin.

Once that pad is on, the unit itself will analyse the rhythm and it assesses the rhythm of the heart.

Assessing heart rhythm. Do not touch the patient. Analysing. Do not touch the patient.

So it's now reading what rhythm the heart's in and it will make a decision.

Shock advised.

So it's now going to shock.

Stand clear of the patient.

We check. Nobody is touching the patient what-so-ever and we make sure all oxygen is removed.

Press the orange shock button now.

We then give the warning, "stand clear, shocking."

Shock delivered.

And the shock is delivered.

Begin CPR.

We then go back to CPR.

It is safe to touch the patient.

Center of the chest, five to six centimetres in-depth, at 120 per minute.

Place overlapping hands in the middle of the chest.

As you can tell, the defib will tell you what to do. It talks you through the whole scenario.

Press directly down on chest in time with the metronome.

Remain calm.

Like having a bird on your shoulder talking to you all the time, telling you what to do.

Good compressions.

It even gives you the speed to do the compressions, which is quite quick, it's 120 per minute. The defibrillator will tell you every step to make once you turn it on. Pad placement, the pads will have pictures on the pads to show you where to put them and as long as you put them in those places and turn the defib on, they will do the rest for you. They will only shock a shockable rhythm. There are four main rhythms the heart is in. Asystole, when the heart is stationary, the defib will not shock that. Normal sinus rhythm, which is what we are all sitting here with, the heart will not shock that. Then there is VF, ventricular fibrillation, and ventricular tachycardia. Both are shockable rhythms. The defib knows what the rhythm is and the defib will only shock shockable rhythms. So you cannot do any harm with it. You won't do anything wrong to the patient.

Also, remember there is always a fall-back position for defib pads. If we have got a patient that has an injury to the top right pectoral muscle or to the left side of the chest, for instance, a knife wound or an injury from a crash that has created the cardiac arrest, the fall-back position is always front and back. So we put one pad on the front of the chest over the heart and the second pad goes on the back of the patient onto the skin, between the shoulder blades, directly opposite the one on the front. And the defib will shock through the chest in that manner. But that is your fall-back plan. The recommended position is always top right, bottom left first. If you cannot get there, your fall-back plan is centre of the chest, between the shoulder blades and shock directly through the chest. CPR alone in a cardiac arrest gives a maximum of around a 10% chance of survival. AED and CPR put together as a team effort, will give you as high as an 85% chance of survival. And I'd far rather have that. So please use defibs.
